At Community Home Solutions we are building partnerships with many GP Practices and Primary Care Networks through our work with Staffordshire County Council. Through these partnerships, we're proactively identifying households where occupants suffer from cold-related illnesses and working with the GPs to swiftly approve eligibility. This means that if you have a long-term health condition which is made worse by the cold, we can speed up the process to access funding to provide you with energy efficiency improvements.

Primary Care Networks we work closely with:
We have been working with the East Staffordshire Primary Care Network and the Seisdon Primary Care Network, alongside the Staffordshire and Stoke-on-Trent Integrated Care Board. We aim to build on our partnerships to broaden our reach further afield than Staffordshire.
